Cervical spine clearance in paediatric trauma: are we clear?

Background and Aims

In adult poly-trauma, cervical spine injuries may be cleared radiologically if normal. However, in children, clinical evaluation has been part of the cervical spine clearance irrespective of radiological clearance due to the risk of spinal cord injury without radiological abnormality (SCIWORA). The National Institute for Health and Clinical Excellence (NICE) published age-appropriate guidelines (under or over 10 years) in September 2007. We aim to survey the current practice of cervical spine clearance in paediatric poly-trauma in the paediatric neurointensive care units within the UK.


Materials and Methods

We identified 17 paediatric neurointensive care units in the UK and performed a telephone survey with either and/or consultant.


Results

All 17 centres answered the telephone questionnaire. Only eight centres (47%) had a written protocol for cervical spine clearance. Cervical spine clearance involves neurological examination (all centres), plain radiography of the cervical spine . . . [Full text of this article]
