Metabolic Acidosis: Differentiating the Causes in the Poisoned Patient
2022, Emergency Medicine Clinics of North AmericaEffectiveness of Sodium Bicarbonate Administration on Mortality in Cardiac Arrest Patients: A Systematic Review and Meta-analysis
2020, Journal of Emergency MedicineCitation Excerpt :The 2010 ACLS guidelines stated that SB can be beneficial in some special resuscitation situations, such as preexisting metabolic acidosis, hyperkalemia, or tricyclic antidepressant overdose (Class IIb, LOE C) (12,13). However, the recommendation was based on small observational studies and animal studies published during the 1980s to 1990s (32–35). Recently, a small randomized control trial reported in 2018 showed that SB administration on severe metabolic acidosis during prolonged cardiopulmonary resuscitation did not improve the rate of ROSC and good neurologic outcome survival (3).
Overdose of cardiotoxic drugs
2018, Cardiac Intensive CareAntidepressant Overdose-induced Seizures
2013, Psychiatric Clinics of North AmericaCitation Excerpt :Phenytoin, although second-line therapy for most seizures, is usually not effective for the treatment of drug-induced seizures or CA overdose–induced seizures.6,81 Patients who are seizing from an antidepressant that causes cardiac sodium channel blockade should be treated simultaneously with 1 to 2 mEq/kg of sodium bicarbonate given as an intravenous bolus in addition to conventional therapy and repeated as needed until a blood pH of 7.55 is attained82,83; sodium bicarbonate is effective for treating drug-induced cardiac sodium channel blockade.84 In addition, lipid emulsion, a novel therapy believed to work by pulling lipid-soluble drugs out of tissues, has been used to successfully restore circulation in a patient who experienced seizure activity and cardiovascular collapse after overdose of bupropion and lamotrigine.85
