Vocal cord hematomas complicating anticoagulant therapy

Described are the cases of two patients who presented with vocal cord hematomas consequent to poor control of anticoagulation. Both patients presented with hoarseness and cough. One required intubation due to respiratory obstruction. Vocal cord hematomas should be considered in patients who present with upper airway symptoms while anticoagulated.

    Sublingual hematoma is the commonest site and occurs in up to 67% of the reported cases. Other reported areas of hematoma include retropharyngeal, submaxillary, epiglottis, and vocal cords [1–5]. Once the diagnosis is confirmed, the mainstay of treatment is to ensure an adequate airway and reverse coagulopathy.

    Vocal fold hematomas in patients on anticoagulation have been described in the literature previously. Kerr and Kwaselow15 described two cases of vocal fold hematomas that occurred spontaneously in patients taking warfarin. Both patients presented with hoarseness and cough, and one patient required intubation to secure his airway.
