Critical Validation of Colour Doppler Ultrasound in Diagnostics of Acute Scrotum in Children

Abstract

Acute scrotum in children is generally a symptom. Several pathologies of the testis, scrotum or groin may cause it, including inflammatory as well as ischaemic processes. Testicular torsion with ischaemia of the testis is an emergency requiring prompt surgical intervention. Even experienced paediatric surgeons and urologists may have difficulties differentiating an ischaemic from an inflammatory aetiology solely on the basis of clinical signs and symptoms. In cases of aetiologic uncertainty, operative exploration has to be performed to rule out ischaemia, which led in the past to a high rate of surgery. More recently, colour Doppler ultrasonography (CDUS) has become an imaging modality for the examination of the acute scrotum with the purpose of detecting ischaemia, thus reducing the need for explorative surgery. In this study we examined the reliability of CDUS, comparing the sonographical diagnoses with both the intraoperative findings and the clinical course.

We present here 132 consecutive cases of acute scrotum in children, who were admitted from 01/98 through 10/02 to our hospital. All patients underwent CDUS. Patients with ultrasonographic signs of ischaemia of the testis (group A) and patients with demonstrated perfusion and suspicious or severe clinical symptoms (group B) were operated immediately. Patients with demonstrated perfusion without suspicious or severe symptoms (group C) were treated conservatively.

Thirty-eight patients (28.8 %) were treated operatively. Eleven cases of testicular torsion in group A (12 pat.) could be confirmed intraoperatively, in one case there was no testicular torsion but a severe inflammation of the testis with torsion of the appendix testis. In group B (26 pat.) operative exploration revealed one case of testicular torsion. In group B two cases of sonographically suspected torsions of the appendix testis also emerged as epididymitis intraoperatively. In the other cases postoperative diagnosis was in agreement with the preoperative CDUS. The clinical course of 94 conservatively treated patients (71.2 %) (group C) confirmed in all cases a non-ischaemic aetiology. There was a total of 12 cases (9.1 %) where acute scrotum was caused by testicular torsions. This relatively low rate underlines the necessity to differentiate between ischaemic and other aetiologies, thus avoiding operation in many patients. 71.2 % of our patients could be treated conservatively. However, it is essential to exclude an ischaemic cause before opting for conservative therapy. Even though CDUS is a very valuable diagnostic tool in this sense, this exclusion cannot be ensured in every case and is highly dependent on the expertise and technique of the investigator. Therefore in cases with any doubts an operative exploration has to be performed to avoid the most dreaded complication in the treatment of acute scrotum.

Objectif

Chez l'enfant, le scrotum aigu est généralement un symptôme signalant un processus pathologique tant inflammatoire qu'ischémique des testicules, du scrotum ou de l'aine. La torsion du testicule avec ischémie testiculaire est une urgence chirurgicale. Néanmoins, sur la base des signes cliniques, même des chirurgiens pédiatriques et urologues expérimentés ont des difficultés à différentier une étiologie inflammatoire d'une étiologie ischémique. En cas d'incertitude étiologique, une intervention exploratrice est nécessaire pour éliminer une ischémie, d'où un taux d'intervention élevé dans le passé. Depuis plusieurs années, l'échographie-Doppler couleur est utilisée pour l'étude du scrotum aigu afin de détecter un processus ischémique, réduisant d'autant le taux de chirurgie exploratrice. Nous avons étudié la fiabilité de l'échographie-Doppler couleur, comparant le diagnostic échographique avec les données peropératoires et les observations cliniques.

Méthodes

Il s'agit d'une série de 132 cas consécutives de scrotum aigu chez des enfants admis à notre hôpital de 1998 à 2002. Une échographie-Doppler couleur était obtenue chez tous les patients. Une intervention chirurgicale d'urgence était réalisée chez les patients ayant des signes échographiques d'ischémie testiculaire (groupe A) et chez les patients présentant une perfusion confirmée et des symptômes cliniques sévères (groupe B). Un traitement conservateur était utilisé en cas de perfusion confirmée sans symptomatologie sévère (groupe C).

Résultats

Une intervention d'urgence était réalisée chez 38 patients (28,8 %). La torsion testiculaire (12 patients du groupe A) était confirmée chez onze patients avec un enfant présentant une inflammation testiculaire sévère sans torsion. Dans le groupe B (26 patients), l'intervention a révélé un cas de torsion testiculaire. Dans ce même groupe B, le diagnostic opératoire était une épididymite chez deux cas de torsion du testicule suspectés à l'échographie. Pour les autres patients de ce groupe, le diagnostic de l'échographie-Doppler couleur préopératoire était en accord avec le diagnostic peropératoire. L'évolution clinique des 94 patients ayant un traitement conservateur (groupe C, 71,2 % de l'ensemble des patients) a confirmé l'étiologie non ischémique. Au total, le diagnostic de torsion du testicule était confirmé chez douze patients (9,1 %) présentant un scrotum aigu. Ce taux relativement faible souligne l'importance de la différenciation entre l'ischémie et les autres causes de scrotum aigu. Dans notre série, une intervention non nécessaire a pu être évitée chez 71,2 % des enfants.

Discussion

Il est essentiel d'éliminer une cause ischémique avant de récuser la chirurgie. Bien qu'elle offre des informations diagnostiques importantes, l'échographie-Doppler couleur n'est pas en mesure de fournir le diagnostic exact chez tous les patients. Elle est par ailleurs hautement opérateur dépendant. En cas d'incertitude, une intervention en urgence reste la seule solution pour éviter les complications graves du scrotum aigu.

El escroto agudo es un síntoma que puede corresponder a varias enfermedades de testículo, escroto o región inguinal en niños incluyendo procesos isquémicos e inflamatorios. La torsión testicular con isquemia es una urgencia que requiere inmediata operación quirúrgica. Incluso cirujanos con experiencia pueden tener dificultades para diferenciar la etiología inflamatoria o isquémica basándose solamente en los síntomas y signos clínicos. En casos inciertos debe ser realizada una exploración quirúrgica para eliminar la isquemia lo que conduce a una cierta contidad de intervenciones innecesarias. Más recientemente la ultrasonografía color doppler (CDUS) se ha convertido en la modalidad de estudio de imagen preferida para el examen del escroto agudo a fin de detectar la isquemia y reducir las exploraciones quirúrgicas. En este estudio examinamos la fiabilidad de la CDUS comparando los diagnósticos ecográficos con los hallazgos operatorios y el curso clínico. Presentamos 132 casos consecutivos de escroto agudo en niños ingresados en nuestro Hospital entre enero de 1998 y octubre del 2002. Todos fueron estudiados con CDUS. Los pacientes con signos ecográficos de isquemia testicular (grupo A) y los que demostraron perfusión pero seguían siendo sospechosos clínicamente (grupo B) fueron inmediatamente operados. Los pacientes con perfusión y sin signos sospechosos (grupo C) se trataron de modo conservador.

Se operaron 38 niños (28,8 %). Once casos de torsión testicular en el grupo A (12 niños) pudieron ser confirmados preoperatoriamente y en uno no había torsión testicular sino una inflamación severa del testículo con torsión del apéndice testicular. En el grupo B (26 casos) la exploración quirúrgica reveló un caso de torsión testicular. En el grupo B se vió que dos casos de torsiones del apéndice testicular sospechadas ecográficamente resultaron ser epididimitis en la operación. En los otros casos el diagnóstico postoperatorio fue el mismo que en la CDUS preoperatoria. El curso clínico de 94 pacientes tratados conservadoramente (71,2 % grupo C), confirmó en todos los casos la etiología no isquémica. En total 12 torsiones testiculares (9,1 %) fueron causa de escroto agudo. La relativamente baja tasa de torsiones subraya la necesidad de diferenciar entre las etiología isquémicas y otras para evitar la operación a muchos pacientes. El 71,2 % de los nuestros pudo ser tratado conservadoramente. Sin embargo sigue vigente que hay que excluir la causa isquémica para justificar el tratamiento conservador. Aunque el CDUS es un arma diagnóstica muy valiosa en este sentido, la exclusión no puede obtenerse en todos los casos y depende mucho de la habilidad y de la técnica del explorador. Por lo tanto, en casos con dudas debe seguir haciéndose una exploración quirúrgica para evitar las temibles complicaciones del escroto agudo.

Zusammenfassung

Das akute Skrotum ist ein Symptom, welches durch entzündliche, traumatische, aber auch ischämische Pathomechanismen hervorgerufen werden kann. Die prinzipielle Möglichkeit einer Ischämie macht das akute Skrotum per se zu einem kinderchirurgischen Notfall, da dann nur durch die sofortige Operation die Prognose günstig beeinflusst werden kann. Die Anamnese wie die klinische Untersuchung geben dabei oft bereits Anhalt für eine bestehende Ätiologie, können aber naturgemäß nicht den Beweis eines nicht-ischämischen Geschehens (Ausschluss Hodentorsion) liefern. Bei bestehender Unklarheit der Ätiologie muss das Skrotum operativ exploriert werden, was in der Vergangenheit zu einer hohen Rate chirurgischer Interventionen geführt hat. Heute steht dem erfahrenen Untersucher die Farb-Doppler-Ultrasonographie (CDUS) als neuere Methode in der Differenzialdiagnostik zur Verfügung. In dieser Studie wird die Rolle der CDUS in der Beurteilung des akuten Skrotums kritisch bewertet und die sonographische Diagnose mit dem intraoperativen Situs wie auch mit dem weiteren klinischen Verlauf verglichen.

Im Zeitraum von 1/98 - 10/02 wurden bei 132 Patienten mit akutem Skrotum die CDUS durchgeführt. Patienten mit sonographisch diagnostizierter Ischämie des Hodens (Gruppe A) oder mit sonographisch nachweisbarer Perfusion aber erheblicher (Schmerz) oder hochverdächtiger Klinik (z. B. Prehnsches Zeichen) (Gruppe B) wurden unverzüglich operiert. Patienten mit sonographisch nachweisbarer Perfusion und milderer Klinik wurden konservativ behandelt (Gruppe C).

Intraoperativ wurden in Gruppe A (n = 12) 11 Hodentorsionen bestätigt. In Gruppe B (n = 26) zeigte sich intraoperativ eine Hodentorsion. Der klinische Verlauf der Patienten in Gruppe C (n = 94) bestätigte ausnahmslos ein nicht-ischämisches Geschehen als Ursache des akuten Skrotums.

In dieser Studie lag nur in 9,1 % der Fälle eine Hodentorsion ätiologisch dem akuten Skrotum zugrunde. Bei 94 Patienten konnte durch CDUS eine explorative Hodenfreilegung vermieden werden. Wenn auch CDUS diesbezüglich eine sehr wertvolle Untersuchungsmethode ist, mit einer Sensitivität von 91,7 % und einer Spezifität von 99,2 %, kann der Ausschluss einer Ischämie nicht immer erbracht werden. Zudem ist diese Methode stark abhängig von der Expertise des Untersuchers. Es gilt weiterhin als Maxime in der Diagnostik des akuten Skrotums der Ausschluss eines ischämischen Geschehens als Rechtfertigung für konservative Behandlung. Deshalb muss bei geringstem Zweifel und entsprechender Klinik auch bei negativer CDUS-Studie die operative Exploration erfolgen, um, wie in unserer Serie, eine Hodentorsion in der Behandlung des akuten Skrotums nicht zu übersehen.
