Clinical introduction
A 57-year-old man presented to the emergency department with pain in left hand over the third metacarpophalangeal (MCP) joint and limited movement of the third finger. The pain started suddenly when he sneezed on the back of his hand. He denied trauma or rheumatological disease. The pain was made worse by flexion and extension of the middle finger.
