Clinical introduction

A 46-year-old man without underlying diseases visited our emergency department because of fever and blurry vision in the left eye for 2 days. General malaise and abdominal fullness were also reported. He denied any trauma to the left eye. Physical examination revealed no specific findings. Laboratory findings showed leucocytosis, high C reactive protein level, elevated liver enzymes (aspartate aminotransferase, 43 IU/L; alanine aminotransferase, 85 IU/L), and hyperbilirubinaemia (total bilirubin, 2.11 mg/dL, and direct bilirubin, 0.99 mg/dL).
