We read Dr Edlow’s comments on the Bestbet ‘Ruling out cerebellar infarct in the emergency department with CT scan’ with interest. We thank Dr Edlow for his helpful input and acknowledge his expertise on this topic. We agree that strict application of some methods of ‘evidence-based’ medicine can indeed fail to provide the whole picture, especially when clinical techniques rely on good evidence of mechanisms or other ‘indirect’ evidence, rather than population level clinical study alone.
