In the last 2 days, she needed to change pads four times a day. No trauma or fever was reported. She was taking fosfomycin for presumed urinary tract infection, without improvement of her symptoms. At physical examination, a red-soft doughnut-shaped mass at vaginal introitus was noted (figure 1).
