Group 1: haemodynamically stable patients diagnosed as having chronic obstructive pulmonary disease (COLD). Group 2: haemodynamically stable patients (defined as administration of inotropic medicine ⩽5 μg/kg/min dopamine) without COLD. Group 3: haemodynamically unstable patients without COLD.
