As a mechanism, this group proposed decreased intracellular adenosine triphosphate, tissue ischemia caused by decrease erythrocyte 2,3-diphosphoglycerate shifting the oxygen dissociation curve leftward, and hypophosphatemic rhabdomyalysis.1 A study of children with kwashiorkor in Malawi found a significant association between severe hypophosphatemia and mortality.2
